The overarching project goal was to move from assessment to investment. This community has a long history in pursuing health improvement and the 13-year regional initiative, Healthy Monadnock, continues to design programs to strive towards being the healthiest community in the nation. In order to take successful strategies to scale, we assumed we had to increase community engagement and sustainable funding for these strategies. As a result of this project, there has been renewed commitment to Healthy Monadnock in the community, a recognition of the need to focus strategies and refine governance to improve performance with action underway to do both, increased capacity of the local staff to support these efforts and development of the business case for increasing population health. We therefore believe that the project identified and began to address the key questions of scale and sustainability. In addition, the strategy and financing tools developed in this project have been used by Wellville communities, leading to the creation of additional business cases for population health. They will also be part of the ReThink Health Ventures toolset and distributed broadly during that project.
